DOG WALKING/ PET SERVICE

I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending walks, pet sitting, training or any activities with Lamour's Club LLC.  I assume all related risks, both known and unknown to me, of my participation in this service. I understand and agree that in releasing my dog(s) in Lamour's Club LLC care,

Lamour's Club LLC has relied upon my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behavior towards any person or any other dog.  I further understand that due to the way that dogs interact with one another, minor cuts and scratches can occur even though the dogs are carefully supervised at all times. While my dog(s) is/are in the care and custody of Lamour's Club LLC, if I am unreachable in the event of an emergency, I hereby authorize Lamour's Club LLC, its agents, and/or representatives to seek immediate veterinary care for my dog. I agree to be financially responsible for any and all costs in connection with,

veterinary, medical or other treatment. I am aware and understand that I should carry my own pet insurance.  I hereby release and agree to save and hold harmless, Lamour's Club LLC it’s directors, officers, shareholders, employees, assistants, members and agents from any and all liability, claims, including claims of Lamour's Club LLC negligence, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Lamour's Club LLC for any and all such liability, claims, suits, actions, losses, injury or damage.

I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of the Application form completed by me. Although we carefully screen all applicants, occasionally we discover

that this is not an appropriate service for every dog. Lamour's Club LLC reserves the right

to permanently remove a dog from our services at any time.

FITNESS CLASSES

I voluntarily consent to participate in any physical activity involved with Lamour's Club LLC. I acknowledge and understand that the Lamour's Club LLC Trainer has the right to stop me from doing any exercise which the trainer sees harmful to me or make me stop

exercising upon observation of any symptoms of distress or abnormal response. I understand that there are potential risks associated with physical activity such as but not limited to: episodes of transient lightheadedness, fainting, abnormal blood

pressure, musculo-skeletal injuries and I assume wilfully to those risks. My obligation to immediately inform Lamour's Club LLC of any unusual pain, discomfort, fatigue, or any other symptoms that I may suffer during and/or after any physical activity.

I hereby release Lamour's Club LLC from any liability with respect to damage or injury (including death) that I may suffer during participation in physical activity during the Lamour's Club LLC class..

GROCERY DELIVERY

I understand and hereby release Lamour's Club LLC of any damage that may be sustained tothe delivery property including, but not be limited to: concrete curbs, sidewalks,

fences, landscaping, sprinkler systems, camera systems, grass, trees, bushes or any

other item, while we are delivering your items onto the property.

CANCELATION / WAIVER / REFUND POLICY

 understand I will be charged the full amount of any service prior to the booking start time, not including the items purchased in the grocery store for deliveries. I agree to cancel any service within 24 hours of the booking start time. Lamour;s Club LLC does not offer refunds of any kind. If you cancel or change your   appointment; any remaining balance will be added as a credit to use within 90 days of the original booking date.

I understand that all items purchased from The Candle Bar are made upon order, and no refunds are accepted.  If your candle arrives damaged, scratched, melted, or bruised, you may send a photograph within 24 hours of the delivery stamp time and date. Lamour's Club LLC, will determine the condition and you may by subject to a replacement.

I waive, release, and discharge Lamour's Club LLC from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for any injuries, or harm (including death), disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity. 

I indemnify, hold harmless, and promise never to sue Lamour's Club LLC or persons participating in any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise
